As an Accredited Social Worker with significant community experience, I provide mobile, home-visiting social work services designed around your unique needs. Telehealth Social Work available for accross Australia for everyone including Rural and Remote clients
My services are available to both self-managed and fully supported Support at Home package clients. I believe in a fully transparent pricing and agreement process before we ever begin working together.
Services can be fully funded by your Support at Home Package with no co-payments as Social Work fits under the Clinical Care Category. I will work directly with your care provider to ensure my services stay carefully within your package budget. This means you get the high-level clinical support you need with absolutely no financial surprises or out-of-pocket costs.
My fee is a flat $225 for clients within 15 kms of my Pakenham base either home visit or telehealth or telephone. My minimum billing time is 15 mins ( ie a phone call).
For clients 15kms to 30kms from Pakenham base cost is $240 an hour
Non direct work such as report writing, emails, is completed at $193 an hour
Initial Visit is 90 minutes direct time with 1 hour report writing

Care Team Integration: I work collaboratively with your Care Manager so everyone is on the same page. I will contact your care partner to arrange the service agreement for onboarding with your provider.
Targeted Interventions: I ensure my psychosocial assessments, capacity building, and advocacy efforts are seamlessly woven into your broader care plan.
