If your package is managed by a provider, my goal is to enhance the care you are already receiving.
Care Team Integration: I work collaboratively with your Care Manager so everyone is on the same page. I will contact your care partner to arrange the service agreement for onboarding with your provider.
Targeted Interventions: I ensure my psychosocial assessments, capacity building, and advocacy efforts are seamlessly woven into your broader care plan.
